Objective: This study aimed to assess the association of extraction of the causative tooth with inflammatory and hematological parameters and short-term clinical outcomes in patients with odontogenic infections.
Material and Methods: This prospective observational cohort study was carried out for six months (April 2025 to September 2025) in the Department of Oral and Maxillofacial Surgery, Liaquat University Hospital, Hyderabad. Using non-probability consecutive sampling, a total of 90 adult clinically and radiographically confirmed odontogenic infection patients were included. Based on treatment approach, patients were assigned into Group-I (n = 45) and Group-II (n = 45). Appropriate statistical tests were carried out for data analysis where p<0.05 was considered significant.
Results: The average age of participants was 36.8 ± 10.4 years and the demographic and inflammatory parameters at baseline were similar between groups. The extraction group had a reduction of 8.2 mg/L of CRP from 16.8 ± 4.2 mg/L to 8.6 ± 3.1 mg/L, while the non-extraction group had a reduction of 5.2 mg/L of CRP from 16.4 ± 4.5 mg/L to 11.2 ± 3.5 mg/L. The extraction group also demonstrated a shorter hospital stay compared with the non-extraction group (3.9 ± 1.2 vs 5.0 ± 1.5 days; p<0.001).
Conclusion: During the acute phase of odontogenic infection management, the removal of the causative tooth was found to be associated with greater reductions in inflammatory markers and improved short-term clinical outcomes.
